Enrolment Procedure

Fill-out the pre-registration online form that can be accessed through the web page’s link.

The Graduate School Office will validate the courses on the pre-registration online form submitted by the student.

The Advising Slip shall then be forwarded to the email address of the student.

The accomplished Advising Slip together with the Clearance shall be presented to the Registrar’s Office.
For New Students, the following requirements must be submitted:
- TOR original copy
- Original copy of PSA Birth Certificate/Marriage Contract
- Two (2) reference form (Kindly download forms by click this link)

The Registrar’s Office will issue the Assessment Form to the student.

The student shall proceed to the Cashier’s Office for the payment of fees.

The official receipt must be presented to the Registrar’s Office for the issuance of the Certificate of Registration.

- Visit the following link for your specific enrolment schedule:
- Proceed to the office of the School of Law located at the ground floor of the new Graduate School building and submit your enrolment documents:
- Original Transcript of Records (TOR)
- Original copy of PSA Birth Certificate / Marriage Contract (for female)
- Two (2) reference form (Kindly download forms by click this link)
- Two (2) pcs passport size colored picture, white background with name tag.
Note: Place the documents in a long brown envelope. Indicate the following information on the top left portion of the envelope:
- Name of Student

Secure the Advising Slip from the same office.

Proceed to the Registrar’s Office and get your Assessment Form.

Pay your tuition and other fees at the Cashier’s Office. (The Cashier’s office will be open from 8:00am - 2:00 pm only).

Go to your respective campuses to submit your enrolment documents.

Proceed to the designated registration counters for the evaluation of the following documents:
- Original copy of Report Card (Form 138)
- Original copy of School Permanent Record (Form 137) with remarks copy for Don Honorio Ventura State University
- Original copy of Certificate of Good Moral Character
- Original copy of PSA Birth Certificate
- Two (2) pcs passport size colored picture, white background with name tag.
Note: Place the documents in a long brown envelope. Indicate the following information on the top left portion of the envelope:

The adviser shall require the students to submit a Letter of Intent (LOI) through their Gmail if they intend to enroll for the 1st semester of A.Y. 2022-2023.Go to your respective campuses to submit your enrolment documents.

In case the Gmail could not be accessed, the adviser shall require the student/s to accomplish the Letter of Intent Form. The final list of students who intend to enroll shall be submitted by the adviser to the chairperson.

The chairperson shall forward the final list of students to the Registrar’s Office on or before July 1, 2022.

The records in charge of the program shall process the enrollment of the students.

The records in charge will provide a list of irregular students to the respective units.

The adviser/chairperson shall:
- check the Academic Program Evaluation at the student portal;
- contact the student for advising which will be carried out online; and
- submit the assessment form to the dean for approval.

The chairperson will then forward the approved forms to the Registrar’s Office.

Submit the "Permit to Shift" Form duly signed by the parent/guardian together with the copy of grades from the last semester attended. (Kindly download form here)

The Office of Admissions shall endorse the student for evaluation to the respective unit where he/she intends to enroll.

The Chairperson shall then evaluate the academic record of the student found in the Student Portal and return the shifting form with remarks approved/disapproved for admission to the Office of the Admissions.
Note: Students with grade lower than 85.0 or equivalent in all enrolled subjects who wish to shift to another academic program with board exam shall not be allowed.

The Office of the Admissions will inform the student about the status of his or her application.

Once accepted, the student shall then bring the necessary documents to the Office of the University Registrar for enrolment:
- Honorable Dismissal/Transfer Credential
- Transcript of Records – original or authenticated copy
- Certificate of Good Moral Character
- Original copy of Birth Certificate issued and authenticated by PSA (Marriage Certificate for married women)
- Two (2) pcs. passport size colored picture in white background with name tag

Submit a Letter of Intent addressed to the University Registrar duly signed by the parent/guardian and the former Dean/Director.

The Dean/ Director shall endorse the student and forward the properly accomplished advising form to the Office of the University Registrar for appropriate action.

For NEW Students

The student/parent must submit all the necessary documents:
- Six (6) pcs 2x2 picture white background with nametag
- Two (2) photocopies of PSA Birth certificate
- Original and 2 photocopies of F9 (Final Report Card)
- Original and 2 photocopies of Good Moral Certificate
- Two (2) Long brown envelopes
- Accomplished the Pre-Registration Form

The name of the student will be checked by the faculty-in-charge (FIC) on the approved list.

The FIC will also check the completeness and authenticity of the submitted documents.

Upon identifying that all required documents are complete and that the form was properly accomplished, signature/s will be affixed on the pre-assessment slip. Such form will be given to the parent or student.

The student/parent will pay the necessary fees at the Cashier’s Office. An official receipt will be issued to confirm this payment.

The giving of Certificates of Registration, Class Schedules, sections, and capturing of ID will be accomplished on separate schedules.
For OLD Students

The student/parent must submit the clearance, Form 9 (final report card), and accomplished pre-registration form to the faculty-in-charge.

The faculty-in-charge will check the completeness of the form.

Once the form is correct and complete, signature/s will be affixed on the pre-assessment slip. Such form will be given to the parent or student.

The parent/student will pay the necessary fees at the Cashier’s Office. An official receipt will be given to the parent or student to affirm the payment.

The giving of Certificates of Registration, Class Schedules, sections, and capturing of ID will be accomplished on separate schedules.

Senior High School
For Grade 12 Students (August 9, 2022)
-
Advisers are instructed to contact the parents of their previous advisory class on or before August 9, 2022 to confirm if their children will enroll for the 1st semester of Academic Year 2022-2023.

The adviser will make a summary report on the day and time the parent was contacted and the date and time the parent affirmed that the learner will enroll for 1st semester of A.Y. 2022-2023.

Once all the parents/guardians confirmed their intention to enroll their children/wards for this school year, the adviser will update the student profile/ directory of the student needed for DepEd’s Learners Information System (LIS).
-
On the scheduled date of enrollment (August 9, 2022), the adviser will forward to the SHS all original Form138 and complete list of Grade 12 students who will enrol for the 1st semester.

The SHS will forward to the Registrar’s Office the complete list and documents of students who will enroll in Grade 12.
For Grade 11 Students (August 10-12, 2022)
-
The parents/guardians of qualifiers shall bring the following during the scheduled date of enrollment:
- original and photocopy of Form 138
- two (2) photocopies of birth certificate
- two (2) photocopies of Certificate of Good Moral
- two (2) pieces 2x2 picture with white background and name tag
- two (2) photocopies of school ID from previous school
- two (2) long brown envelopes and QVR/ESC Certificate

On the day of enrolment, the faculty-in-charge (FIC) will check the student’s name on the List of Qualifiers issued by the Admissions Office.

Once the FIC have verified that the student belongs to the list of qualifiers, he/she will then get from the parent/guardian the original and photocopy of Form 138, two (2) photopies of birth certificate, two (2) photocopies of Certificate of Good Moral, two (2) pieces 2x2 picture with white background and name tag, two (2) photocopies of school ID from previous school, and two (2) long brown envelopes, and QVR/ESC Certificate if from private school.

After the required documents were collected from the parent/guardian of the expected enrollee, the FIC will then consolidate the documents and forward them to the University Registrar.

For non- voucher recipients, the parent/s will be advised to proceed to the Cashier’s Office to pay the tuition and miscellaneous fees.

The start of the 1st Semester of A. Y. 2022-2023 for ALL LEVELS (School of Law, College, Senior High School and Laboratory High School) will be on August 22, 2022.
