The ISO remote 2nd Surveillance Audit was conducted at the Don Honorio Ventura State University on November 03-05, 2021 to ensure that the University complies with the ISO standards and provides its clients-the students, the parents and other stakeholders quality and prompt services in the areas of instruction, research, and extension.
ISO Remote 2nd Surveillance Audit was conducted at the Don Honorio Honorio Ventura State University last November 3 to 5, 2021 to asses the Institution’s compliance to the prestigious ISO Standards. The aforementioned audit became a platform to exhibit DHVSU’s noble mission of delivering service with paramount quality to clienteles and stakeholders in reference to areas of instruction, research, and extension.
The three-day surveillance audit was kicked off with a short opening program where Engr. Rohel S. Serrano, the Executive Vice President, delivered the welcome remarks while Dr. Enrique G. Baking, the University President, articulated the purpose of the activity. It was followed by the introduction of the different personalities who composed the Quality Assurance Team and the External Auditor by Dr. Emmanuel B. Parreno, the Director for Quality Assurance.
The three-day surveillance audit kicked off with a brief opening program via virtual meeting. Engr. Rohel Serrano, the Executive Vice President, welcomed the participants. Dr. Enrique Baking, the University President, emphasized the importance of the surveillance audit. It was then followed by Dr. Emmmanuel B. Parreno, the Director for Quality Assurance, who introduced the University’s Quality Assurance Team.
After the brief opening program, Dr. Shahid Ali Mushtaq, the External Auditor from the Russian Register, together with the University’s top officials and the Internal Auditors proceeded with the different phases of the surveillance audit such as updates on the processes, quality policies, improvements, and other aspects of the organization including risks management and opportunities.
The Russian Register’s External Auditor, Dr. Shahid Ali Mushtaq, facilitates the surveillance audit via virtual conference. The QA IT Team provided technical assistance during this three-day event.
As part of the audit, Dr. Mushtaq interviewed Dr. Gloria B. Gigante, the Director of the Office of the Student Affairs, on a number of items such as Product Realization/ Safe Products Planning and Realization, Control of Production and Service Provision, Control of Non- Conformities, Identification and Traceability, Preservation, Inspection and Testing, Sales Process, Job Completion Process, and Consultation and Testing Process.

Usually, intelligent and challenging questions are expected from external auditors. As the case, Dr. Robin B. Dimla, the Director of the Research Management Office, with his professionalism and expertise, had extensive discussions with Dr. Mushtaq on the processes in the Research Management Office.

Dr. Mushtaq likewise interviewed six (6) different staff of the University from the Learning Resource Center and the Administrative Services Office.
Considering that the DHVSU Extension Campuses in Porac and Sto. Tomas underwent their surveillance audit last November 20, 2020, Dr. Mushtaq decided to audit the Lubao Campus for this 2021 Surveillance Audit.
Taken into account their surveillance audit last November 11, 2020, DHVSU’s extension campuses in Porac and Sto. Tomas were no longer audited. Instead, Dr. Mushtaq assessed the DHVSU Lubao Campus for this said audit.
On November 5, 2021, the said audit was concluded with a program where Dr. Mushtaq gladly announced that the University passed the 2nd Surveillance Audit. This was then followed by the pledge to commitment to sustain and continuously improve the QMS in the University. This was delivered by Engr. Reden M. Hernandez, Vice President for Academic Affairs.
The highlight of this surveillance audit during the Closing Programme on November 5, 2021 was the announcement of Dr. Mushtaq that DHVSU passed. Everyone in the virtual meeting gave a resounding sighs of reliefs followed by congratulations when Dr. Mushtaq flashed on his screen the word PASSED as the official result of the 2nd surveillance audit.
